Clover Park Technical College (“the College”) has created a method and procedure to receive reports of alleged misconduct or other potential violations. Unless you identify yourself, all reports received by the College are confidential and anonymous. The College values your right to privacy on all reported information and will not knowingly disclose any information that would identify you without your express permission, unless required to do so by law. If you wish to remain anonymous, please do not report information in a manner that may personally identify you. For example:
• Do not include your relationship to persons identified in your report
• Do not include your physical location relative to persons or incidents in your report
The college only uses the information submitted by reporters to investigate the issues reported. By submitting a report, you authorize the college about which you are filing a report, and its affiliated and subsidiary entities, to collect, process, store and otherwise use the report and information submitted as the College deems appropriate.
RELEASE
By submitting a report to the college, you, the reporter, understand and acknowledge that the sole obligation of the college is to review your report. You, on behalf of yourself or your heirs, successors, family members, agents and assigns, agree to and do hereby release the College and its respective officers, directors, shareholders, employees, successors, predecessors, agents and assigns, of and from any and all liability, claims, actions, causes of action and damages which relate in any way to our client’s actions or failure to act as the result of having received, or having access to, information relative to a report. This system is not a formal complaint process, and a report to this system is not a substitute for a formal complaint pursuant to the appropriate College policies and procedures.
ACKNOWLEDGEMENT
The college takes the reports made through this site seriously. Please make your report as accurate and truthful as possible. Do not intentionally include false or misleading information in your report. The intentional reporting of false or misleading information can result in civil or criminal liability.
